F5 is needed because the picture is in your browser cache.
Someone who did not yet visit the page will directly see the new pictures, as the old one does not exist any more on the site nor in his cache.
If you wait few days you'll also see the new ones without pressing F5, as the images in your cache have a date limit (else imagine the size of that cache)

I was wondering if the main pictures for previously viewed pages that have been replaced ever update automatically, or does one always need to refresh/F5?

Along with that, how would someone know that the pictures they are seeing are actually the current main image without needing to refresh every opened page. This is in the interest of not duplicating someone else's efforts or having someone else upgrade already upgraded pictures.
